Qualifications:
• Bachelor of Science in Physical Therapy from an APTA-accredited program
• Active state license as a Physical Therapist (or multi-state PTLC license, if applicable)
• 2 years of experience in acute care, rehabilitation, or home health preferred
• Strong therapy skills aligned with industry standards and applicable regulations
• Proof of current CPR certification
• Excellent communication and clinical documentation skills
• Knowledge of durable medical equipment
• Reliable transportation and current auto liability insurance
• Valid driver's license
Essential Functions:
Patient Care & Treatment
• Performs ongoing clinical assessments, including OASIS, to identify patient needs
• Develops, evaluates, and revises the plan of care in coordination with the patient, family, and physician
• Delivers treatments for neuromuscular, neurological, and orthopedic conditions
• Supervises PTAs and HHAs per state and agency standards
• Implements and follows infection control protocols during patient care
Communication & Collaboration
• Communicates effectively with patients, caregivers, and the interdisciplinary team
• Participates in case conferences and shares updates with clinical leadership
• Educates patients and families on therapy plans and expectations
Documentation & Compliance
• Prepares and completes documentation, clinical notes, and phone orders in a timely manner
• Ensures documentation meets regulatory, payor, and agency requirements
• Maintains compliance with clinical policies and procedures
Professional Standards & Development
• Accepts assignments consistent with professional training and competency
• Maintains current licensure and CE requirements
• Demonstrates ongoing professional growth and participates in QAPI activities
Environmental and Working Conditions:
• Possible exposure to blood, bodily fluids, and infectious diseases
• Ability to work flexible hours and travel locally
• Some exposure to outdoor elements and unpleasant weather
Physical and Mental Effort:
• Requires lifting, bending, pulling, pushing, standing, and walking for extended periods
• Ability to work under pressure to meet deadlines and patient needs
• Requires hand-eye coordination and manual dexterity
Continuing Education:
• Attends in-service and educational programs
• Meets continuing education requirements for state licensure
