Qualifications:
- Physical Therapy Assistant, licensed per the state requirements
- 2 years of Home Health preferred
- Good communication skills, assessment skills, and strong clinical documentation skills.
- Ability to read, write, and speak the English language
- Reliable transportation. Valid and current auto liability insurance

Essential functions:
- Treats patients as directed by the Physical Therapist.
- Records treatments administered in medical record.
- Records clinical documentation according to accepted regulatory, corporate and professional guidelines.
- Participates in weekly case conferences.
- Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
- Provides verbal or written patient status updates to supervising PT prior to reassessment and discharge visits, and as indicated by patient response to treatment.
- Participates in in-service training programs.
- Responsible for assuring patient safety.
- Complies with state regulations and national standards as they apply to the practice of physical therapy.
- All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
