🏡 Job Summary
As a Physical Therapist (PT), you'll evaluate patients, create and carry out treatment plans, and coordinate with our rehab team to provide top-tier care in the home setting.
🛠️ Key Responsibilities
- Evaluate patient’s condition and functional level
- Develop and implement treatment plans
- Supervise PTAs and HHAs per plan of care
- Provide in-home therapy and adaptive equipment guidance
- Maintain accurate and timely documentation
- Ensure patient and family education for home programs
- Submit notes/visits in a timely manner
- Adhere to HIPAA confidentiality standards
