Position Summary
The Prevention Coordinator II (PC2) provides direct prevention services and community engagement for The Movement, CrescentCare’s CDC-2203 funded program prioritizing Black MSM (ages 13–34). The PC2 conducts HIV/STI/HCV testing and counseling, PrEP education, and sexual health outreach, while assisting with health campaigns, data collection, and client navigation. This role is central to building trust, engaging young Black MSM, and linking clients to prevention and care services, while ensuring compliance with CDC-2203 deliverables, NHM&E data entry, and program requirements.
Key Duties and Responsibilities
- Provide client-centered HIV/STI/HCV counseling, testing, outreach, and referrals to high-risk individuals in field and clinical settings
- Deliver PrEP education and referrals for HIV prevention
- Maintain confidentiality and supportive counseling during pre-/post-test sessions
- Coordinate culturally relevant outreach, education, and engagement with young Black MSM in community, in program safe space, and digital spaces
- Actively participate in community events and mobile outreach operations including set-up and breakdown
- Support development of health messaging, campaigns, and recruitment strategies, ensuring materials are approved by the program review panel
- Maintain accurate data entry, NHM&E reporting, and compliance with CDC-2203 deliverables
- Assist in hosting focus groups, events, and community discussions
- Maintain condom distribution sites and prevention materials
- Represent program in schools, community groups, and youth-serving organizations
- Assist with data entry, reporting, and documentation required by CDC
- Participate in CDC-approved trainings on HIV testing, NHM&E, and evidence-based interventions
- Work in collaboration with Movement team lead, Prevention outreach team programs, internal linkage teams, CrescentCare staff and providers, and mental health navigators to ensure access to wraparound services
- Other duties as assigned by Team Lead or Program Manager/PI
Qualifications
Education
- Bachelor's degree and 1 year of experience in public health, harm reduction, or social services; or
- High School Diploma/GED and 3 years of relevant experience
Other (licensure, certifications)
- Experience with MSM populations, sexual/reproductive health, HIV/HCV services, or peer navigation
- Valid Louisiana Driver’s License required
- Willingness to work flexible hours, including evenings and weekends
- Prior HIV/HCV testing and counseling certification (or ability to complete training within 60 days)
- Bilingual Spanish a plus
ESSENTIAL PHYSICAL REQUIREMENTS
- Extended periods of sitting required.
- Work is primarily office based.
- Some travel may be required.
- May need to lift up to 25 pounds in equipment and supplies
