GENERAL PURPOSE or JOB.

The admissions Coordinator is a member of the management team involved in resident planning and policy.

STANDARD RDQIJIRDMENTS

  • Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment
  • Supports and cooperates with specific procedures and programs for:     9k=
  • Safety, including universal precautions and safe work practices, established fire/safety/disas{er plans, risk management, and security, report and/or con•ect unsafe working conditions, equipment repair and maintenance needs
  • Confidentiality of all data including resident, employee and operations data

       9k=    Quality Improvement and compliance with all regulatory requirements

9k= Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal discriminatory behavior 9k= Supports and participates in common teamwork:

  • Cooperates and works together with all co-workers

2Q==Uses tactful, appropriate communications in sensitive and emotional situations

  • Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns
  • Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned

ESSENTIAL DUTIES AND RESPONSIBILITIES.

  • Provide a home like, warm reception to all potential residents, designees and families        9k=
  • As Assessments come in give to Director of Nursing or designee for approvaE Z ReferraE sheets filled out during tour or via phone with resident or their designee 2Q== Obtain a Meca check for all residents to determine payer source Z If managed care, check benefits and co-pay.

2Q==Verify Hospital Days

ZVerify if resident has been at any other facilities that would affect their Medicare days available

2Q==          Verify if resident has any outstanding balances with the business office 2Q==           Assign Room Doctor

2Q==If coming from another SNF, AL or home must receive 3008 first

  • Assign medical record number — write in book

2Q==      Have consent to treat signed upon admission,

  • If family is out of town send packet certified mail and follow up and document conversations with families.
  • Track all referrals, who referred and outcome. Refusals must state why refused (i.e. medically complex, no appropriate bed, etc),
  • Copy assessment to Rehab and Nursing,
  • Make Armband" door tag — Roommate form
  • Put DNR on gold paper
  • Podiatry form faxed with face sheet to KG Health Partners (Podiatrist)

    2Q==   If patient has never been in facility before, fax PASSR to Cares Unit

    2Q==    Fax/Scan DNR to doctor for signature

  • Ensure Resident Surveys have been mailed out
  • Give Social Services a copy of the face sheet within 24 hours
  • If changes are made to the Face Sheet circle changes and give new copy to business office 9k= Daily Census report
  • Assists in managing changes to face sheets
  • Check payer source totals to census report — clinical reports/clinical census/payor/enter date/run
  • Fax census to Pharmerica with face sheets
  • Scan/Fax census to department heads
  • DCF Medicaid info copy to Laura Weber- place original in file
  • Hospice financial sheet —sign, date Scan/fax to business office original Tn File
  • Any other changes repeat above — if room move make arm band — only financial changes go to business office

    9k=    Facilitate move in process for new residents as necessary, checking rooms, etc.

Z Coordinate with the Director of Marketing and other Oak Manor staff on joint marketing plans. Z Serve as the liaison between the Administrator, medical, professional and supervis01Y staff and residents and their families at admission

  • Assist Concierge in following residents to ensure proper adjustment to new surrounding.
  • Assist with minor Social Work issues.
  • Understand financial questions and concerns as it pertains to Medicaid, Medicare, and Secondary Insurances.
  • When Nurse Liaison is unavailable, visit residents in the hospital.
  • Walk through empty rooms daily and repolt issues to proper departments.
  • Attend daily department head meeting, updating staff on room status
  • Prepare a monthly Admit/Disoharge report to track and frend the following:
  • New admissions Z      Re-admissions
  • Dischargeé
  • Referral Sources for new admits
  • Refused or Deblined
  • Home Health Company Patient Discharged to

  • Average daily census for the month, overall, Medicare, and Managed

EDUCATION

Bachelor's degree (B. A.) from four-year college university; or two to four years related experience and/or training; or equivalent combination of education and experience.

EXPERIENCE

Two to four years related experience and/or training

LANGUAGE SKILLS.

Ability to read, analyze, and interpret general business periodicals, professional journals, technical 2Q==or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Abiiity to effectively present information and respond to questions fi•om groups of managers, clients, customers, and the general public,

MATHEMATICAL SKILLS.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume, Ability to apply concepts of basic algebra and geomet1Y.

CONIPUTDR SKILLS

Ability to use Microsoft programs such as Word, Excel, PowerPoint, and Outlook with proficiency. Able to use various software programs to create marketing and posted materials.

RDAS 0NNG ABLITY.

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete val'iables.
