Job Title: Staff Pharmacist 

Department: Pharmacy 

Reports To: Pharmacy Director 

FLSA Status: Exempt 

Summary: 

Pharmacist will serve as a key team member and resource for improving patient care and outcomes for patients at NOHN supporting its mission.  Pharmacist will maintain good working relationships and cooperate actively with NOHN leadership team, Health Services Department Management, Healthcare providers, and staff; and other companies outside associates to ensure that the objectives of the department and organization are met. The Pharmacist will achieve this goal in the following roles: 

Consultation/Collaboration: 

  • Responsible for managing customer interactions in the pharmacy, including patient counseling, complaint resolution, quality assurance management, and adherence to practices which promote customer satisfaction.  

  • Provide asynchronous chart review for Comprehensive Medication Review for patients with polypharmacy to evaluate for reconciliation, drug interactions, lab monitoring needs and ordering, evidence-based medicine recommendations. 

  • Complex case management review as requested by providers/prescribers. 

  • Provide recommendations regarding dose and drug modifications based upon patient clinical characteristics or drug-drug interactions, including assistance and advice on dose adjustments and tapering schedules. 

  • Conducts special assignments as requested by Pharmacy Director.  

Refill Management: 

  • Ensuring proper processing of all third-party prescriptions and perform all necessary functions in the system in accordance with policies and regulations.  

  • Coordinate and authorize refill requests from patients and pharmacies with attention to appropriateness of therapy, time intervals, and patient adherence to clinical follow up visits 

Education: 

  • Serve as resource for providers and staff education regarding guidelines, FDA alerts, new drug information and Evidence-Based Medicine support, drug interactions, side effects, dosage and storage of pharmaceuticals. 

  • Assist in the development and implementation of programs for patients and staff. 

  • Precept residents, students and NOHN staff on matters pertaining to pharmacy. 

Collaboration: 

  • Report prescription incidents immediately and adhere to company policies and procedures relating to prescription incidents and the Quality Improvement Program.  

  • Attend staff meetings to advise and inform NOHN medical and dental staff of medication applications and related policies, as requested.  

  • Actively participate in administrative committees and focus groups as requested.  

  • Cultivate relationships with pharmacy organizations aligned with the NOHN mission (e.g.: pharmacy schools, grade schools, local pharmacy societies). 

Administrative: 

  • Responsible for the utilization of technology, automations, and management of workflow within the pharmacy department.  

  • Manages the improvement and maintenance of the pharmacy department housekeeping, maintenance, and appearance for safety, health, and well-being of the customers and employees; assures adherence to applicable laws and government regulations.  

  • Manage integrity of medications within NOHN system, including storage and expiration dating. 

  • Assist in medication inventory management. 

  • Maintain established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste pharmaceuticals. 

  • Uphold and implement procedures in pharmacy according to NOHN policies and procedures. 

  • Practice pharmacy specific and general organizational infection control policies and procedures. 

  • Maintain up to date immunizations, as required of clinical staff. 

  • Abide by Health Insurance Portability and Accountability Act of 1996 in reference to all confidential information obtained through job tasks. 

Dispensing: 

  • Responsible for safely filling and dispensing prescriptions in a clinical setting.  

  • Direct pharmacy staff engaged in preparing medications for dispensing.  

  • Observe pharmacy personnel at work and help develop quality assurance techniques to ensure safe, legal and ethical practices. 

Supervisor Responsibilities: 

  • This job currently holds no supervisory responsibilities but may be required over time as program and needs grow.  

  • In absence of Pharmacy Director, help supervise Pharmacy Technicians, Pharmacy Assistants, Technicians in training, and PT Pharmacists. 

Qualifications

  • Education: Bachelor's degree in Pharmacy or Doctor of pharmacy (PharmD) degree,  Current license to practice pharmacy in the State of Washington,  Maintenance of any applicable registrations/certifications needed to successfully complete the job duties assigned, including immunization certification, Basic Life Support (BLS) certified and renewed on an annual basis, Valid Washington State Driver’s license, Preceptor licensure is encouraged upon hiring 

  • Experience: Completion of an ambulatory care residency OR 2-5 years of Retail/Outpatient pharmacy experience preferred. 

  • Skills: Proficiency in EHR systems, Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plan and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.  Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.   

  • Language: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures or governmental regulations. Ability to write routine reports, business correspondence, and procedures manuals. Ability to effectively present information and responds to questions from managers, clients, customers and the public.
