Pay: $23.00/hour
Schedule: Monday-Friday 8:30 a.m. - 1:00 p.m. or 9:00 a.m. - 1:30 p.m.

Job Summary: The Housing Solutions Case Manager, under the direction of the Housing Solutions Manager, provides ongoing assessments, personal housing and emergency plans, support, referrals, and advocacy for individuals in the Mercy House Permanent Supportive Housing Program. This role supports residents in building self-reliance, promoting positive behavior change, and addressing obstacles to housing stability. Accurate recordkeeping and a trauma-informed, client-focused approach are essential.

Essential Duties and Responsibilities:
Client/Service Delivery

  • Maintain a caseload of 20–25 individuals; meet with residents at least twice monthly to develop emergency and housing support plans with measurable goals
  • Use harm reduction and client-centered methods to promote housing stability
  • Apply crisis prevention and intervention skills
  • Practice Trauma-Informed Care and Motivational Interviewing
  • Implement eviction prevention strategies
  • Help residents build on strengths and assess needs respectfully
  • Conduct home visits and ensure units are maintained
  • Connect clients to community resources as needed
  • Facilitate social and recreational activities to promote peer support
  • Coordinate with volunteers and community partners for employment, education, tutoring, and mentoring services
  • Attend relevant trainings (e.g., co-occurring disorders, harm reduction, cultural competence)
  • Support client–landlord relationships
  • Participate in the emergency on-call schedule

Team/Staff Relations

  • Collaborate with staff and volunteers to support the agency’s mission and values
  • Attend agency meetings and contribute to resident program planning

Administration

  • Maintain accurate, up-to-date documentation (housing/emergency plans, case notes, assessments, call logs, housing authority documents)
  • Complete paperwork promptly to ensure agency reimbursement
  • Assist with data collection systems
  • Willingness to learn about Adult Rehabilitative Mental Health Services and Substance Abuse Treatments
  • Perform other duties as assigned

Qualifications

Requirements:
Education and Experience

  • Bachelor’s degree in social work or related field preferred, with experience in chemical/mental health and co-occurring disorders
  • Candidates without a degree but with at least 2 years of relevant experience and knowledge of mental health and addiction are encouraged to apply

General

  • Valid CA driver’s license, insurance, and reliable vehicle required
  • Comfortable conducting home visits and meeting clients in various settings
  • Strong communication, organizational, crisis intervention, and interpersonal skills
  • Dependable and professional in diverse environments

Physical Requirements

  • Ability to lift up to 50 lbs
  • Prolonged periods of desk and computer work
