Qualifications

What You’ll Do

• Provide health oversight, medication management, and wellness coordination for individuals in supported living programs.
• Collaborate with interdisciplinary teams to create holistic care plans.
• Educate direct support staff on health and safety protocols.
• Monitor medical appointments, follow-up care, and health documentation.
• Serve as an advocate for each person’s health and quality of life.
