General Description

Shared Wellness is founded on the belief that people living with intellectual and developmental disabilities are not defined by their diagnosis and, with the proper support, are able to achieve their dreams and become valued members of their communities.

Shared Wellness is currently seeking for dedicated LPNs to work in our home base setting with an individual that are Trach, G- Tube and Vent dependent.

Role and Responsibilities

 

Duties and Responsibilities

 

  • Ensure the well-being of the consumer.
  • Provide appropriate care in accordance with personal centered plan and all physician order.
  • Provide home care nursing to adult consumer in home and community setting.
  • Create a self-centered environment for the  individual - focused on his/her hopes, dreams, and goals.
  • Follow all care plans, ISP, and protocols.
  • Ensure that individual’s home and medical equipment is clean and safe.
  • Lead for insuring individual’s health maintenance

 

Required Skills

  • Excellent Documentation skills
  • Proficient in G-Tube and J-Tube care and medication administration
  • Ventilator experience
  • Tracheotomy experience
  • Use of appropriate policies and systems as required for this position.
  • Compassionate, caring, and punctual

 

 

Qualifications and Education Requirements

  1. Current unencumbered nursing license (LPN) in state of PA
  2. Current CPR
  3. Minimum 1 year LPN homecare experience
  4. Minimum 1 year Trach, Vent, and G Tube experience
  5. Graduate of an accredited nursing program

 

Preferred Skills

  • Communication Skills (written and verbal)
  • Computer skills/data entry

 

  • Knowledge of 6400 (CLA) and 6500 (Life Sharing) PA ODP Regulations
  • High degree of willingness to learn, ability to change and be flexible based on situational requirements
  • Organizational Skills and Multi-tasking
  • Attention to detail
  • Reliable and able to meet deadlines with minimal prompting
  • Person-centered thinking experience or training
  • Creative
