Hours: Full Time or Part Time (2nd and 3rd Shift Positions)

Overview
The care worker will provide client care as required under the supervision of the licensed nursing staff. The care worker completes assigned tasks to ensure timely, efficient and safe client care. Communication and interactions with clients and their families is an essential component of this position.
Duties and Responsibilities:
- Performs initial and ongoing data collection on all clients under the direction of the Registered Nurse/Licensed Practical Nurse.
- Implements clients’ plans of care as developed and updated by the Registered Nurse
- Provides direct client care (ADLs) and reports condition and changes to RN or on call nurse.
- Assures resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
- Assists clients to different parts of St. Ann Center as needed.
- Participates in Bi-annual CPR recertification, staff meetings and other in-services as required.
- Maintains neat and orderly environment in the unit. Housekeeping as necessary to keep rooms and unit clean.
- Reports any needed supplies to the supervisor or team lead.
- Documents results of client’s care within established policies and procedures and scope of practice and reports any changes in client conditions to other shifts.
- Prepares and serves meals, assist with feeding as necessary.
- Oversees the volunteers as needed/requested.
- Answers phones.
- Passes meds once training is completed.
- Participates in field trips when needed.
- Work in other units as designated by the supervisor.
- Provides other duties as assigned and requested by management.
- Complies with all policies and procedures.
- Assists supervisor in providing for the personal hygiene, social and recreational needs of the respite care clients.
- Performs daily tasks assisting with the nutritional needs, personal care needs and ambulation as designated by the supervisor.
- Gives individual attention to clients as needed.
- Attends staff meetings and team huddles, makes oral or written reports identifying any client problem areas, (social, recreational, spiritual, psychological or emotional) and/or the improvement of services. Also attends and participates in professional seminars, workshops and all staff meetings.
- Report any incident or quality of care concerns to the supervisor, on call nurse or director.
- Report any maintenance issues to the supervisor.
- Takes on call along with other staff.
Varied Hours (Must be flexible) -
7:00am-3:30pm
3:00pm-11:30pm
10:00pm-6:00am
All shifts are full time with every other weekend. Extra shifts are possible.
5-6-week rotation of on-call for finding staff coverage.
