POSITION SUMMARY:
The Patient Navigator serves as a key resource for patients by providing guidance and support in accessing health coverage and community resources. This role focuses on assisting patients with enrolling in health insurance programs such as Texas Medicaid, the Health Insurance Marketplace, and other applicable financial assistance options. The Patient Navigator educates patients on their eligibility, assists with applications, and ensures that they understand the steps required to maintain coverage.

In addition, the Patient Navigator collaborates with clinical and administrative staff to reduce barriers to care, improve patient access to services, and support continuity of care. The ideal candidate will demonstrate strong communication skills, cultural sensitivity, and a commitment to helping patients navigate complex healthcare systems.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Duties may include, but are not limited to:

  1. Assist patients with understanding and enrolling in health insurance programs, including Texas Medicaid, SNAP, Marketplace Insurance, and other financial assistance programs.
  2. Educate patients on eligibility requirements, documentation needed, and the application process.
  3. Provide one-on-one support to patients in completing applications, submitting required documents, and resolving issues with insurance coverage.
  4. Maintain accurate records of patient interactions, application submissions, and follow-up actions.
  5. Collaborate with clinical and administrative staff to ensure patients receive timely access to care.
  6. Maintain regular communication with patients, confirming appointments, conducting follow-ups, and supporting adherence to treatment plans.
  7. Identify and address barriers to care, such as transportation, language, or financial constraints, by connecting patients to appropriate community resources.
  8. Stay current with changes in Medicaid, Marketplace, and other insurance program policies and requirements.
  9. Participate in team meetings, training sessions, and quality improvement initiatives.
  10. Assist with medical receptionist activities as needed.

Qualifications

QUALIFICATIONS:

  • High school diploma or equivalent required
  • Bilingual in Spanish Preferred
  • Texas Community Partner Program Site Navigator Certification, Preferred
  • Experience working in a healthcare setting or social services environment preferred
  • Knowledge of Medicaid, Marketplace Insurance, and other health coverage programs preferred
  • Strong interpersonal and communication skills
  • Ability to work effectively with diverse populations and maintain patient confidentiality
  • Proficient in Microsoft Office and experience using electronic health record systems preferred
