The Jericho Vocational Services Center (JVSC) utilizes a “one-stop” model for providing vocational and educational training, mentoring, case management, supportive and linkage services to create successful transitions to employment and career pathways. Jericho provides services for any Los Angeles County resident 18 years and older. Additionally, our Jericho program specializes in vocational services tailored for individuals impacted by the criminal justice system.

The Community Health Worker will provide wraparound Intensive Case Management services that are individualized, survivor focused, and trauma informed to systems-impacted women (those that have encountered the criminal or juvenile justice system, i.e. arrested, charged with, or convicted a criminal offence, or by felony probation), to improve health outcomes and reduce recidivism rates.  These Services include but are not limited to: Referral and linkage to physical health, mental health, and substance use treatment services, housing support services, employment and education services, cognitive behavior interventions, and other supportive services based on client need. In addition, Community Health Worker Lead will serve as a Liaison with program administrator, program manager, and assigned Justice Care and Opportunities Department (JCOD) program manager to ensure effective operation and delivery of high quality POWR program services.

 

Primary Duties

  • Support and ensure effective operation and delivery of high quality POWR program services.
  • Process and accept referrals through outreach and engagement, community-based organizations, Diversion and Collaborative Courts, Sherriff’s Department, Jail In Reach Providers, Probation and Parole departments.
  • Complete eligibility screening and enroll clients into database.
  • Assess all incoming referrals to determine program eligibility.
  • Conduct intake and assessment activities with clients including assisting with gathering other program eligibility documentation, completing intake forms, entering screening, assessment, and encounter information into database.
  • Obtain all appropriate consents and authorizations for client’s participation in POWR for staff to communicate effectively with partners and other service providers to assist clients with linkages to resources, services and benefits acquisition, coordination of services, client advocacy, and other needed services.
  • Conduct bio-psychosocial assessments and establish a reentry care plan. The care plan will address the needs identified in the comprehensive psychosocial assessment and describe client’s goals, action steps to reach goals, timeframes for completing goals, and disposition of each goal as it is met or changed.
  • Provide cognitive behavior interventions through case management visits to identify helpful and unhelpful behaviors and develop skills to solve problems and adopt new behaviors to facilitate effective re-entry.
  • Maintain regular ongoing client contact and tailor the intensity of services provided, including the frequency of face-to-face visits, according to client\s level of functioning and acuity of needs.
  • Accompanying clients to medical and mental health appointments, as well as court/parole/probation appearances, when appropriate.
  • Assistance with benefits establishment (including completion and submission of applications for health insurance benefits).
  • Housing navigation (e.g., interim housing assistance)
  • Assist client with obtaining basic needs, such as clothing, food, transportation as well as gaining, restoring, improving and/or maintaining daily independent living skills and community participation and social/ leisure.
  • Assist clients with locating and securing employment, job/vocational training and or volunteer opportunities.
  • Monitor and follow up with clients and service providers to confirm timely completion of referrals and linkages, access to services, and maintenance of services.

Qualifications

Requirements

  • Be proficient in reading, writing in English.  Bilingual Spanish (a plus).

  • Possess a working knowledge of conducting interventions for clients with a history of substance abuse, incarceration, mental illness or homelessness.

  • Working knowledge of client eligibility screening for entitlement program (DPSS, Voc. Rehab, Cal Works, etc.)

  • Working knowledge of compatible service systems including, but not limited to, DPSS, SSI ,etc

  • Know and comply with Agency policies and procedures, HIPAA.

  • Comfortable working with a diverse population including LGBT, formerly incarcerated and people with co-occurring disorders.

  • Present ideas, information, and viewpoints clearly, both verbally and in writing.

  • Efficiently use the personal computer to include a variety or word processing software.

  • Utilize analysis, experience, and judgment to make decisions.

  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.

  • Adapt to changing needs by acquiring new skills and knowledge.

  • High school diploma or GED

  • A valid California driver's license is required for this position, as driving is an essential function of the role and is required for field work, client crisis response and emergencies. A current automobile insurance is required.

  • It is required that the Community Health Worker possess “lived experience” to meaningfully connect with this population. It is a desirable qualification to have been incarcerated for a continuous period of six months.

  • Provides services within a culturally sensitive model of practice and is respectful to the community and clients.

  • COVID-19 vaccination required. May request medical or religious exemption if applicable.
