SUMMARY:
The Case Manager will work in our Hopwa Family and Serenity Homes and be responsible for ensuring client safety and providing care coordination and case management services in an effective and efficient manner for all clients.
Work Schedule: Full time, Monday - Friday, 9:00 a.m. - 5:00 p.m.
RESPONSIBILITIES:
- Provide supportive counseling to clients and their family
- Healthcare Advocacy for client with Infectious Disease Provider, psychiatrist, mental health providers and providing escort to appointments
- Facilitate and attend case conferences with other service providers
- Conduct assessment, reassessment and service plans tailored to meeting client’s needs, providing appropriate referrals, advocacy, and home visits
- Attends and assist in preparation of Team meetings and Community meetings
- Provides education to client on healthcare promotion and individual living skills
- Provides monthly apartment inspection and document in progress notes
