ROLE SUMMARY:
The Peer Partner works closely with clients to represent their best interest. They support with processing referrals and assist with services including triaging applicants, providing supportive community resources, and skill-building activities.
ESSENTIAL RESPONSIBILITIES:
- Conduct skill building with individuals, caregivers, families, and groups.
- Provide referrals to supportive community resources.
- Process online and walk-in referrals which includes gathering and uploading required Medi-Cal, DMH, and Wellnest clinical documentation and IDs ensuring these items are completed before referring for clinical triage. Referral paperwork will from a variety of referral sources (e.g., Schools, phone calls, emails, walk-ins, SRTS)
- Reach out to applicants to with enrollment process, and foster support and trust.
- Create new client records in Agency’s Electronic Health Record (EHR).
- Work with Access clinical staff on ensure timely triage, assessment, and case assignment of referrals.
- Track status of referrals and report case assignment needs to Access and Wellness Program Manager to promote stability for client.
- Maintain completion of dispositions on electronic tracking systems (e.g., SRTS, SRLs, and walk-in logs).
- Complete phone or in-person triages (billing for TCM, completing screeners, and PSC/CANS)
- Assisting with other administrative duties assigned.
REQUIRED QUALIFICATIONS:
High School diploma (or equivalent) required. At least 2 years of experience with entering and tracking of computerized data preferred. Proficiency with software including Microsoft Word and Excel preferred.
CONDITIONS OF EMPLOYMENT:
Employee may be asked to pursue additional training when it is determined to be in the best interest of the Company. Employee must have the ability to use a personal vehicle with the required insurance coverage to complete the essential duties of their job and conduct agency business. Employee must maintain a valid California driver’s license and clean driving record that meets Wellnest’s insurance carrier’s criteria for liability coverage. In keeping with Company policy for all employees, the successful applicant must also complete a test for TB, and fingerprint clearance by the Department of Justice.
WORKING CONDITIONS:
Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening and/or weekend hours for time-sensitive projects.

- Lift and/or move up to 25 pounds.
- Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required.
