Description

This position reports directly to the Intensive and Diversion Services Program Administrator within the BH division. This position will perform assessments, recovery planning, resource navigation, referrals to appropriate community services, and outpatient behavioral health services to include crisis services as needed to individuals with severe and persistent mental illness involved in criminal justice system through the Texas Correctional Office of Offenders with Medical or Mental Impairments (TCOOMMI) program. This position requires the use of initiative and independent judgment to provide interventions in the community, home and school to individuals on caseload.

 

This position provides intensive case management services, in accordance with the TCOOMMI Program Guidelines, intended to stabilize the individual’s mental health symptoms, reduce the risk of recidivism, and aid in the individual’s increase awareness of and participation in community and natural supports, development of skills in self-advocacy, tenure in the community, and ability to participate in independent mental health care. The position ensures services provided are person-directed to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. This position provides person-directed skills training and psychosocial rehab to develop coping and other skills needed to feel successful in their home and community. This position must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes and can work independently and collaboratively as needed and have effective communication with supervisory staff when necessary.

This position practices good stewardship of resources by ensuring all required trainings are completed prior to the due date and center-issued equipment is kept in good repair.

 

Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and Lifepath Systems policies and procedures. This position will work closely with Adult Fidelity managers and the Program Administrator to ensure that all gaps are reconciled.

 

This position builds positive relationships with local Parole and Community Supervision and Correction Department offices. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities are required.

 

Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve. This position must have the ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner.

 

Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of half of time worked, with each individual receiving at least 1 community-based service monthly. 90% of service notes are signed within 2 business days of service. 75% of full chart reviews receive a score of 80 or higher in following the Golden Thread. Ensures all new referrals are seen within 7 days from release or referral date, that the supervising officer is made aware of all appointments, cancellations, or rescheduled appointments/attempts, the client’s financial status is reviewed monthly, the recovery plan is reviewed every 90 days, and all members of the IDT team participate in recovery planning, and communicate with the supervising offices at least monthly. Must conduct a face-to-face visit with the individual within seventy-two hours of receiving notice of an arrest and/or detention of an assigned individual. Provides person directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services with a focus on access to care and diversion from incarceration. Must understand criteria for LifePath Systems services and be able to explain to individuals served as well as community partners.

Qualifications

  • Bachelor’s degree in psychology, counseling, social work or related field
  • Three (3) years of experience in the treatment of individuals with mental illness, substance use disorders, and/or criminal justice involvement. If a shorter time, the individual’s experience, and aptitude to learn and perform role quickly will be taken into consideration.
  • Experience in Texas Recovery and Resiliency (TRR) required
  • Knowledge of Trauma-Informed Care Practices required.
  • Must have reliable transportation with a valid driver’s license and auto insurance.
  • Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention and symptom management services to individuals with serious mental illness or serious emotional disturbance.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services. 
  • Ability to write clearly and professionally on all services and internal communications.
  • Ability to think clearly and logically while dealing with potentially dangerous and/or emotional situations.
  • Ability to respond to individuals with patience, empathy and understanding.
  • Organized, detail-oriented, and computer-proficiency.
