Essential Responsibilities:
- Developing a confident, reliable, and trusting relationship with the patient(s) and clients we serve
- Assist with medication management across multiple providers, specialties, and diagnosis
- Weekly patient touchpoints
- Post-visit follow-up for all health-related appointments
- Phone/virtual, or in-person appointment availability
- Coordinate and monitor patient-specific, comprehensive care plans encompassing treatment plans across multiple providers, specialties, and diagnosis
- Assist in coordinating all of One to One health services to meet the needs of selected patients
- Assist in educating patients on prescribed treatment plans, and coordinate answering patient questions related to their treatment plans
- Attend patient appointments as needed/requested
- Facilitate communication between all specialty groups
Requirements
- Excellent verbal and written communication skills
- Experience with EMR systems
- Adaptable in differing situations
- Able to multi-task, work independently, and self-driven
- Must be flexible with hours/shifts
- Always demonstrates professionalism
- Displays teamwork with a team-player mindset
- Organized with attention to detail
- BLS Certification
Minimum Qualifications
- Licensed as an RN
- Prior Case Management and/or Patient Navigation experience preferred
