JOB SUMMARY
The Health Federation seeks a qualified, reliable, and detail-oriented individual with a background in community health, relationship-building and project support to serve in this Full-Time position.
The Project Assistant will work with the Integrated Peer Models for Health (IPMH) team on a program funded by the Pennsylvania Department of Human Services. This program supports capacity-building within the North Philadelphia Health Enterprise Zone (HEZ) to benefit populations facing multiple barriers to health and wellbeing, including mental health, and/or substance use challenges, social drivers of health, and chronic disease.
The Project Assistant will play a supportive role on the IPMH team and will primarily work with two components of the program: Connect4health.org, a resource and referral platform and our Peer Models for Health Learning Collaboratives, a professional development-focused community for Community Health Workers/Peer Professionals and their supervisors.
To be successful, this individual must be very organized and a confident communicator who is skilled in written and verbal communication. This individual must be a reliable and consistent team member who is collaborative, eager to learn, and adept at using new technologies. The most successful candidate will also have experience working in direct-service community health roles in clinical or community-based settings.
JOB SPECIFICATIONS
Responsibilities/Duties
Learning Collaborative (50%)
● Attend, participate in, and support learning collaborative in-person and virtual sessions Support activities include keeping attendance and monitoring the chat during virtual sessions.
● Manage learning collaborative session registration and evaluation processes: Create and send out registration forms, evaluations, and participation certificates, as well as timely emails to participants ahead of and following learning collaborative sessions.
● Ensure learning collaborative participant list is kept up-to-date and accurately track participation ● Interview new learning collaborative members and advisory board applicants ● Work with Program Manager to coordinate and facilitate Learning Collaborative Advisory Board meetings
● Attend external CHW and Peer trainings and regional collaboratives to stay up -to-date on relevant workforce trends
Connect4Health (50%)
● Attend relevant findhelp/Connect4health training webinars and opportunities to enhance learning and apply learning where possible
● Facilitate Connect4health trainings with priority community organizations
● Support the Program Manager in identifying and make meaningful connections to valued programs and services
● Utilize Connect4health platform activity data to support outreach and engagement with Community Based Organizations (CBOs)
● Support Program Manager in running Connect4health advisory group meetings
● Create and distribute newsletters to Connect4health partner organizations and their users
● Ensure Connect4health user lists are kept up-to-date
● Contribute to program monitoring, evaluation, reporting, and improvement activities
● Represent the Health Federation of Philadelphia in internal and external meetings/calls as needed
● Assist with documentation of minutes and partner follow up communications
