Required Qualifications
- Associate degree in behavioral science, Social Work, Public Health, or related field and 3 years of experience in outreach/navigation/case management for individuals experiencing homelessness or behavioral health.
OR
- Bachelor’s degree in related field and 2 years of experience in outreach/navigation/case management.
- Minimum 2 years of supervisory or leadership experience.
- Experience with HMIS and/or electronic health records.
- Commitment to participant wellness, housing-focused modalities, and trauma-informed practices.
Preferred Qualifications
- Master’s degree in human services, Social Work, or Public Health.
- Lived experience of homelessness and/or SUD/Mental Health recovery.
- Experience with PATH, SAMHSA, or similar grant-funded programs.
- Bilingual or multilingual skills (Spanish, Somali, Hmong, or other languages).
Physical Requirements
- Ability to walk up to two miles per day in varied environments and weather conditions.
- Ability to lift up to 25 lbs.
- Must have a valid driver’s license, insurance, and access to a personal vehicle (mileage reimbursement provided).
