PHYSICIAN: MD or DO, BC/BE Internist, Geriatrician, or Family Practitioner with Hospice & Palliative Medicine being a desirable distinction – Subacute Skilled Nursing Facility / Geriatric Palliative Care.
Location: Colorado Springs, CO
Organization: Rocky Mountain PACE
Position Type: Full-Time Monday–Friday | No Nights or Weekends | Shared Group Call
Â
About Us:
At Rocky Mountain PACE we believe healthcare should care for providers as much as for participants. We are a mission-driven organization offering comprehensive, interdisciplinary care to older adults, with a strong commitment to promoting holistic wellbeing for both our participants and our staff. As part of the national PACE (Program of All-Inclusive Care for the Elderly) model, we help medically complex seniors age with dignity — in the setting they call home.
Â
Position Summary:
We are seeking a compassionate Physician to join our subacute Skilled Nursing Facility (SNF) Transitions Team, focusing on geriatric palliative care and goal-concordant care. This role is perfect for a Physician with prior hospitalist or SNFist experience who is looking for a meaningful career with exceptional work-life balance.
Â
Key Highlights:
• Monday through Friday schedule — enjoy your weekends and evenings!
• Rotating (2:30) shared Group call coverage from home using internal Nurse Coordinator triage                                                                                                                                                                                            • Supportive interdisciplinary team (IDT) collaboration with physicians, nurses, therapists, social workers, Pharm D, and others.
• Focus on whole-person wellbeing — addressing not just medical, but emotional, psychosocial, and spiritual needs.
Â
Â
Â
Core Responsibilities:
• Provide high-quality subacute and palliative care management for complexly ill participants admitted to contracted SNFs – census of 20-25 patients, averaging 5 admissions +/- discharges per week, and allowing for hybrid on-site/remote work with bedside rounds typically 2-4 times per week.
• Lead and participate in IDT meetings to develop and implement individualized, participant-centered care plans frequently addressing palliative and end-of-life needs.
• Engage in meaningful advance care planning and goals-of-care conversations.
• Coordinate transitions of care to optimize outcomes and respect participant preferences.
• Partner closely with families, SNF teams, and the larger PACE clinical team to ensure seamless, compassionate care.
• Contribute to ongoing quality improvement initiatives and a culture of kindness, respect, and excellence with all partners.
Â
What We’re Looking For:
• BE/BC Physician licensed  in  COLORADO with active DEA registration.
• Minimum 2 years of experience as a hospitalist, SNFist, or in post-acute/long-term care settings.
• Expertise in geriatric syndromes, chronic disease management, palliative care, and interdisciplinary care models.
• Passion for person-centered, holistic care and commitment to working collaboratively within an IDT.
• Strong communication and leadership skills; bilingual ability is a plus.
Â
Why This Role Matters:
• Healthcare is personal. At PACE, you are part of a community that sees the whole person, not just the patient.
• You make a daily difference. Your clinical judgment, leadership, and heart directly shape participant lives.
• We support you too. Enjoy a predictable Monday–Friday schedule with no nights, no weekends, and the lightest call imaginable, so you have time to nourish your own wellbeing.
Â
Why Join Us?
• Predictable, flexible schedule that supports your personal wellbeing in a hybrid work environment
• Competitive salary, generous paid time off, and full benefits package.
• Work in a collaborative, mission-driven environment where your voice matters.
• Opportunities for professional growth in geriatric and palliative care leadership.
Â
Your Impact:
• Serve vulnerable older adults who rely on PACE as their primary source of medical care.
• Provide hands-on medical care and thoughtful symptom management in SNF settings, focusing on quality of life and dignity.
• Partner with an interdisciplinary team — physicians, nurses, social workers, rehab therapists, dietitians, chaplains — to design and deliver holistic, individualized care plans.
• Lead goals-of-care discussions, advance care planning, and family support activities.
• Guide participants through challenging transitions with compassion, advocacy, and skill.
• Contribute meaningfully to a care model that addresses the social, emotional, spiritual, and medical needs of every participant.
Â
If you are seeking a career that fulfills your professional purpose and personal calling, we would love to meet you. And when you’re not making a difference in the lives of older adults, you can enjoy all that Colorado’s Rocky Mountain lifestyle has to offer — from hiking, skiing, and biking to vibrant local culture, food, and community. It’s the wonderful balance of meaningful work and inspired living.
