We are a nonprofit Program of All-Inclusive Care for the Elderly (PACE) serving older adults who live with serious illness, frailty, and socioeconomic hardship. Our mission is to care for the whole person—body, mind, and spirit—through an interdisciplinary team (IDT) approach that centers on dignity, equity, and compassion.

About the Role
We are seeking a curious and collaborative Nurse Practitioner to join our PACE IDT as part of a robust interprofessional care model. You’ll work hand-in-hand with PCP partners, nurses, social workers, rehab therapists, behavioral health specialists, dietitians, recreation therapists, chaplains, and others to deliver wraparound care that engages older adults as they age in place, on their own terms.

What You’ll Do
• Provide whole-person primary and supportive care in the clinic, home, and community-based settings.
• Partner with participants and families in managing chronic and serious illness, goals of care, and care planning.
• Co-lead IDT discussions, contributing your voice to shared decision-making.
• Participate in quality improvement and professional development initiatives guided by physician mentors and coaches.
• Foster trust, presence, and accountability across all disciplines.
