- A Bachelor of Social Work degree (BSW) required, or related experience and/or combined experience required.
- A minimum of one year of experience working with frail or elderly populations is highly desired.
- Experience working on an interdisciplinary team preferable.
- Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
- Ability to use Word computer program, Outlook Calendars and Email
- Experience documenting in an Electronic Health Record preferred.
- Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
