Schedule: This position is a part of the Nursing Management Team and will regularly work weekdays with rotating on-call and Manager on Duty. This position must be willing to work the floor as needed as well.

Role:
We are seeking a dedicated Admissions/Social Services Director (RN or LPN) to join our team! The Admissions/Social Services Director makes assessment for, plans and advocates for the residents of the Community so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions/Social Services Director will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.

Responsibilities:
- Follows and upholds the Community’s philosophy and goal to maintain, improve and/or enhance each resident’s quality of care and quality of life
- Perform pre-assessment of residents as necessary.
- Attends and participates in facility meetings and in-service training programs as assigned.
- Participates in the nurse on-call rotation
- Promotes positive public relations with residents, family members, guests, and outside referral sources.
- Collects and assesses data relevant to resident’s psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.
- Completes assigned portions of the MDS.
- Maintains referral tracker.
- Coordinates admissions and discharge planning
- Reports monthly discharges to the Ombudsman.
- Coordinates and directs the care plan meetings.
- Facilitates advanced directive decision-making process
- Coordinates grievance complaint process
- Coordinate and assist with resourcing and scheduling of allied health services i.e. Dentists, Podiatrists, Medical specialists etc.
