Role:
We are seeking a dedicated Assisted Living/Memory Care Nurse to join our team! The Float Nurse is responsible for the provision of tenant services, including assistance with personal care, medications, social and recreational activities, meal services and other tasks as needed to facilitate the continued well-being of tenants. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.

Responsibilities:
- Conduct pre-admission assessments to evaluate each resident’s functional, cognitive, and health status prior to occupancy.
- Develop, implement, and maintain individualized resident service plans that reflect current needs and preferences.
- Observe and recommend reassessment for residents demonstrating a significant change in condition or functional ability.
- Ensure all assessments and documentation remain current and compliant with state regulations.
- Oversee all clinical care and procedures within the Assisted Living and Memory Care communities, ensuring high standards of practice.
- Participate in the hiring, training, delegating, supervision, and retention of staff.
- Participate in the on-call rotation with other nurse leaders, responding to clinical needs or emergencies as assigned.
- Perform on-call responsibilities as directed by the Director of Clinical Services.
- Performs all other duties as assigned by the Executive Director and/or designee.
