RN Clinical Manager/QA Nurse - Home Health - Full Time

 

The RN Clinical Manager/QA Nurse is responsible for assessing, planning, coordinating, and evaluating the clinical and operational functions to ensure quality, cost-effective home health care is provided by a multidisciplinary staff. Responsible for the administration of the clinical services of the agency and ensure compliance with regulatory, reimbursement, and accreditation standards. Responsible for the review of documentation, all OASIS visits and OASIS submissions.
 

Responsibilities Include:

  • Analyze, review and report on charts, charting trends, and quality assurance indicators.
  • Assist the quality department with surveys and audits as assigned. Participate in agency QAPI program through review of quality measures, patient and agency outcomes, chart audits, development of agency goals and performance improvement plans.
  • Ensure compliance with Medicare conditions of Participation and assist in preparing for accreditation surveys.
  • Assist with professional education activities to encourage professional skill proficiency for all agency clinicians.
  • Review and approve all documentation submitted by Licensed Professionals for evidence of medical necessity, skilled intervention and adherence to the POC, provide education as indicated.
  • Review OASIS scoring for accuracy, provide education to clinical staff regarding appropriate scoring of OASIS, prepare OASIS for submission
  • Monitor compliance to policies and procedures including but not limited to clinical documentation.
  • Scheduling of visits upon approval of MD orders, admissions, evaluations
  • Assist with case conference, coordinate with internal and external clinical staff regarding changes in the patients status.
  • Obtain, review and approve orders.
  • Oversight of patient care.
  • Assist with day-to-day office and staff management related to patient care.
  • Participate in on-call rotation.
  • Make patient visits and/or supervisory visits as needed.
  • Assist with referrals and insurance authorizations.
  • Assist with orientation of new staff.
  • Other duties as assigned.

Qualifications

Job Requirements:

  • Must have an active RN nurse license in MA
  • Solid understanding of the federal, state, and local regulatory guidelines governing the operation of a Home Health Agency.
  • Minimum of 2 years of home healthcare experience (or the equivalent in experience).
  • At least 1-year experience in quality improvement preferred or directly related field.
  • Accurate data entry skills.
  • Knowledge of medical terminology
  • Familiarity of ICD-10 coding. Certification preferred.
  • OASIS experience required. Certification preferred.
