Description

OBJECTIVE/PURPOSE: 
Under the direction of the Program Coordinator - Supported Independent Living Program (SILP), the Direct Support Professional - SILP works to support, encourage, and teach independence in all areas of daily life to adults with a developmental disability. 
 
MAJOR RESPONSIBILITIES: 
 
1.0     HOME SUPPORT: 
 
Teach daily living skills in the consumers’ homes and in the community. Assist consumers with health care, finances, cooking, shopping needs, home care, personal care, and community education. Role model and provide education with healthy relationship skills and boundaries. Serve as a consumer advocate with medical, dental, legal professionals, and other service providers, as needed. Monitor with proficiency self- medication administration, as needed. Demonstrate ability to respond to consumer emergencies and provide emotional support, as needed.  
 
2.0     COMMUNITY SUPPORT: 
 
Advocate on behalf of consumers with family, peers, and community members, as needed. Introduce consumers to community resources and facilitate usage. Encourage and support participation in community events. Teach consumers safety skills in all aspects of community and home life. Teach, instruct, and role model appropriate community and social behavior and expectations, as needed.  Participate as a contributing member of the Supported Independent Living Program, Developmental Services, and UCS teams.    
 
3.0     RECORD KEEPING / ADMINISTRATIVE: 
 
Submit daily time sheets using correct codes for services provided in our electronical medical record system. Request and submit all necessary documentation from other service providers and professionals. Complete progress notes, monthly summaries, and all relevant documentation in a timely manner. Maintain awareness with all relevant consumer goals and supports as stated in the ISA, and document progress. Attend staff meetings, supervision, and scheduled agency/division meetings, as required. Attend all necessary required trainings.  
 
* In addition to the above, performs other duties as assigned. 

 

Qualifications

EDUCATIONAL REQUIREMENTS: 
High School Diploma or equivalent.  Relevant college courses preferred. Computer skills including e-mail usage. 
 
LICENSES/CERTIFICATIONS REQUIRED: 
Valid Driver’s License
