Description

Do you love coordinating and scheduling appointments and you are interested in the healthcare field? If so, CTF Illinois has the perfect position for you as a Healthcare Coordinator around the Chicagoland Suburbs! At CTF Illinois, we provide services for adults with developmental disabilities. The objective of this position would be working directly with nurse’s taking those we serve on appointments and being an advocate for them.

 Job Duties: 

  • Work hours should coincide with Day Program operation hours, which are typically Monday – Thursday, 8 AM – 5 PM, Friday 8 AM – 4 PM, but may be subject to change, per supervisor request.
  • Check monthly for appointments (i.e. medical, vision, dental, neurological, etc.) that are due to be scheduled for residents. 
  • Schedule and confirm appointments, coordinate transportation needs, and accompany individuals on all appointments.
  • Follow-up as necessary and appropriate.
  • Assist nursing to check in monthly medications if requested or needed.
  • Ensure that a medical synopsis form (completed by the nurse) is taken to the appointments as appropriate
  • Return all appropriate documentation to nursing for follow-up.
  • Ensure physician orders are signed by physician monthly
  • Complete bi-annual medication review form and ensure physician signature is obtained.
  • Adhere to terms and conditions of employment as outlined by Employee Handbook and job description.

Qualifications

Requirements: 

  • High School Diploma or general education degree (GED).
  • Valid Driver's License with acceptable driving record
  • Basic user knowledge of Microsoft Office (Excel, Word, Outlook,etc.), and/or other PC applications.
  • Prefer one year working experience directly with individuals with developmental disabilities.
  • Previous DSP certification preferred.
