We’re hiring clinical social workers to join our team using the Collaborative Care Model (CoCM)—a proven, team-based approach that integrates mental health into primary care. It’s designed to make care more accessible, coordinated, and effective, especially for conditions like depression and anxiety.

Position Summary:
The Behavioral Care Manager (which is our term for Clinical Social Worker) is one of three clinicians who collaborate for each patient. The BCM works in conjunction with the Primary Care Doctor and a Psychiatric Consultant who works for MindHealthy. The BCM will provide support through assessments, care planning, and regular follow-ups to help individuals improve their mental health. This includes using proven tools to track progress, adjusting care plans when needed, keeping accurate records, and working closely with our psychiatric team and other healthcare providers to ensure coordinated, effective care.
