Position Summary: 

The Behavioral Care Manager (which is our term for Clinical Social Worker) is one of three clinicians who collaborate for each patient. The BCM works in conjunction with the Primary Care Doctor and a Psychiatric Consultant who works for MindHealthy. The BCM will provide support through assessments, care planning, and regular follow-ups to help individuals improve their mental health. This includes using proven tools to track progress, adjusting care plans when needed, keeping accurate records, and working closely with our psychiatric team and other healthcare providers to ensure coordinated, effective care. 

Essential Functions: 

  • Help support the mental and physical health of a group of assigned patients. 

  • Work with patients to understand their needs, make care plans, and connect them to helpful services. 

  • Provide short-term mental health support using proven methods like motivational interviewing or problem-solving strategies. 

  • Monitor patient progress through an internal EMR system and watch for any changes in their symptoms. 

  • Work with the Psychiatric Consultant to support medication management, including checking on side effects and how well the treatment is working. 

  • Join weekly meetings with the care team to review and update patient care plans. 

  • Identify patients who may need more advanced mental health care and make sure they get the right support. 

  • Handle other related duties as needed. 

Competency: 

  • Strong written and verbal communication skills. 

  • Exhibit a positive, flexible and solution-focused attitude. 

  • Ability to quickly synthesize medical and behavioral health data and formulate effective and evidence-based clinical recommendations. 

  • Maintain active licensure within state(s) of jurisdiction. 

  • Strong knowledge of clinical standards of care. 

 

Qualifications

EXPERIENCE / EDUCATION REQUIREMENTS:

  • Completion of MSW accredited program.
  • Current LCSW, LCMHC or LCAS license within state of jurisdiction. Provisional is acceptable.
