Purpose of Job:
The purpose of this position is to provide comprehensive Clinical Pharmacy Services to patients. To work collaboratively with healthcare providers to ensure the safe and effective management of medication therapy. Effectively communicate with clinic staff to share knowledge, skill and drug expertise. The Clinical Pharmacist participates in all aspects of the medication-use system to provide individualized pharmaceutical care and to help ensure quality outcomes for patients.
Essential Duties and Responsibilities:
- Review clinical data to identify patients and patient populations in need of medication therapy management or clinical pharmacy services
- Review patient EMR data to assess and evaluate prescribed medications
- Create and maintain positive and effective communication with providers and healthcare teams to ensure quality care for individual patients
- Participate in patient consultations to discuss medication history, effectiveness, appropriateness, side effects, and compliance
- Perform individualized consultation for specific patients as referred by primary care providers
- Provide chart notes from patient consultation to the primary care team including but not limited to overview, detailed medication utilization plan and appropriate evidence-based recommendations.
- Participate in patient monitoring and follow-up, review patients progress to identify, prevent or mitigate drug related problems
- Assure pharmacy is an integral part of the health-care delivery system and facilitate enhancement of clinical pharmacy services and programs
- Actively identifies practice related issues which require evaluation and facilitate clinical projects or quality improvement initiatives
- Develop and present medication education to providers and other healthcare personnel as needed to advance practice
- Perform medication therapy management reviews in Outcomes
- In the absence of other pharmacists, participate in pharmacy operations, medication dispensing and counseling in the pharmacy setting.
- Perform other duties as assigned
