Description

Job Summary:    Provides direct patient care to adult and geriatric populations, under the guidance and direction of the Registered Nurse.  Job responsibilities include patient hygiene, patient mobility, vital signs, and blood glucose monitoring, as well as various housekeeping and cleaning duties to maintain a safe, clean environment.

Job Responsibilities:

  1. Admits/Discharges/Transfers patient and completes appropriate forms
  2. Provides for patient hygiene
  3. Provides for patients food and fluid needs
  4. Assists with patient mobility
  5. Obtains and records vital signs
  6. Maintains safe environment of care
  7. Provides for appropriate pressure ulcer prevention
  8. Performs finger stick blood glucose monitoring under the direction of the RN
  9. Completes documentation accurately and in a timely manner according to hospital policies and procedures
  10. Provides age-appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to primarily adult, and geriatrics populations as indicated
  11. Supports patient’s rights and responsibilities, including respect of cultural, ethical, and religious diversity.
  12. Adheres to standard precautions and infection control standards.

Qualifications

Education Requirements and Other Requirements:

 

                Education Level: High school diploma or equivalent

 

Certification/Licensure:  Current Nurse Assistant certification by the State of Illinois, documentation of previous Nurse Assistant certification within the past 5 years, and/or documentation of on the job training in a patient care setting equivalent to nurse assistant training preferred.

 

              BLS certification required

 

Experience Requirements:   Long term care, rehabilitation, and/or medical surgical care experience preferred

               

 

 

Working Conditions: Exposure Category I:  Tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or a potential for spills or splashes of them, care Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
