INTRODUCTION:

The Life Skills Coach is responsible for the direct supervision, training/teaching, and care of individuals within assigned program, community settings or within the program.  The life skills coach will implement Individual Support Plans and act as a resource for individual’s family and team member.

 

ESSENTIAL PROGRAM DUTIES (other duties may be assigned):

Act as a role model by displaying proper and safe dress, language, safe work practices and a positive work attitude.

Support individuals in: work attitude, skills, personal care, socialization, behavioral skills, self-advocacy, following daily schedules and other areas as needed.

Facilitate group and individualized activities geared towards assisting Individuals served to learn new skills by presenting curriculum in a positive and enthusiastic manner while in the community and program settings. 

Implement teaching strategies geared towards helping the Individuals served increase their independence and achieve goals identified by them and their ISP team.  Work with the Sr. Life Skill Coach and Program Dir. to provide feedback relating to the Individual’s strengths, challenges and interests to the Inter-disciplinary Team. Work as part of the larger team to maintain a stable and consistent learning environment.

Provide input and feedback to the Senior Life Skills Coach toward developing a dynamic and engaging schedule geared towards providing meaningful activities that meet the needs and interests of the Individuals served.

Follow community safety guidelines in regards to providing individual oversight and safe transportation in all settings.  Drive company vehicles including large passenger vans using Charles river policies as well as state laws, i.e. use of cell phones prohibited when driving, etc.

In conjunction with the Sr. Life Skills Coach and the Case Coordinator, conduct needs assessments on each Individual served.  Utilize the information drawn from theses assessments to assist the Individual and their team in building a person centered plan and determining new goals. 

Implement support strategies developed by the Individual’s ISP team to assist the Individuals in meeting their goals and objectives.  Maintain accurate and consistent data collection in line with the Support Plan’s guidelines.

Monitor individuals at lunch, breaks and cab duty and during transportation.

Provide back-up support with job coaching for individual employment.  Provide direct support during group employment activities.  Encourage community integration and support community relationships.

Maintain trainings and adhere to company policies and procedures.  Be responsible for company property.

Work with individuals and staff to ensure that shared spaces are organized, efficient & safe. Keep aisles and means of egress clean and accessible and report unsafe situations.   Maintain a clean and organized environment within the program, off-sites & the vans.  Perform light housekeeping and maintenance as required.

Work with other staff & management to ensure the Health & Safety of all individuals.  Notify manager of health and safety concerns and facility issues immediately.

Participate in learning opportunities provided by the Program Director and Clinical Director in relation to Positive Behavior Supports.  Implement support strategies that follow PBS guidelines, and work as part of the larger Clinical team to ensure proper and consistent implementation of Behavior Plans.

Crisis intervention including: consistently carrying out positive behavior plans; intervening in crisis situations; proper implementation of Behavior Management, Emergency Restraint interventions, accurate & timely completion of incident reports & other related paperwork.

Have thorough knowledge of and always uphold Human Rights Policies

Complete all required documentation daily/weekly, etc in a timely fashion; individual timesheets, attendance, cab duty forms, fire drill forms, ISP data sheets, positive behavior plan data collection, community/curriculum forms, and other various forms needed for safety and maintaining individuals records, etc.  

Have thorough knowledge of and become efficient in using computer software programs: to include, but not limited to Epay, Icentrix, and ETS.  Utilize Epay appropriately for documenting work time as well as time off requests.

Attendance:  Adheres to program schedule and routines, which includes arriving to assigned location promptly and conforms to the agency’s attendance standards.  Attend all required meetings and trainings promptly.

Initiative:  Self-reliance in thinking, planning, and carrying out job responsibilities.  Demonstrates problem solving skills.

Management:  Supports and enforces agency policies and objectives; sets example through personal conduct and performance.

LANGUAGE SKILLS:

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write advanced reports and correspondence.  Ability to communication effectively with members served in their primary mode of communication, members under supervision, employees of the organization, families and members of the community. Ability to effectively present information individually and in small groups.

 

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. Employee must have significant endurance to sustain physical demands of the job.

While performing duties of the job, the employee is regularly required to sit, stand, walk, stoop, kneel or crouch.  The employee is frequently required to use hands/fingers to handle objects, tools or controls and to reach with hands and arms.  The employee must frequently be able to generate the force to maneuver up to 100 pounds.  Assistance in lifting members will be provided whenever possible.  Employee must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.  

 

REASONING ABILITY:

Ability to carry out detailed written or oral instructions.

Qualifications

QUALIFICATIONS:

  • High School Diploma required
  • Experience in the Human Services field preferred.
  • Successful completion of all state and agency mandated trainings.
  • Knowledge and use of principle of Normalization and Community Inclusion.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Disability Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily.  Reasonable accommodations may be made to enable members with disabilities to perform these essential functions.
  • Driver’s License that is valid in Massachusetts and reliable personal transportation.
  • Proficient computer skills in Microsoft applications and software programs used by the program
