What You’ll Do:

The person in this role will coordinate the services for young adults in transition. This may include working with various agencies, providers, faith based organizations, and individual partnerships. They will develop, implement, and manage the individualized plans for young adults ages 16-24. They will help to ensure young adults are provided with the tools, support, and resources necessary to become self-sufficient.

Qualifications

Education and Experience

  • Minimum of a bachelor’s degree in social work or a related human service field
  • 2 years’ experience working with transition age youth and/or individuals affected by homelessness, mental health or substance abuse.
  • Requires general knowledge on “Housing First” philosophy and strategies.
  • Possess strong organizational skills with ability to meet a demanding workload.
  • Detail-oriented to complete requirements of files and maintain contract compliance.
  • Excellent communication skills, particularly listening, mediation, and writing skills.
  • Creative thinker/adaptive personality.
  • Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths-based” case management a plus.
  • Demonstrated knowledge of community resources, social service agencies, and landlords.
  • Fluency in the English language is required. Spanish speaking ability is an added asset but is not required.
  • Experience with computer and knowledge of Microsoft Office.
