SUMMARY:

The Hospital-Based Clinician will be placed in the hospital, with varying departments to provide immediate, comprehensive, and coordinated care. The core objective of this role is to engage behavioral health patients in a timely manner, offer essential motivational and behavioral support, and facilitate seamless transitions to ongoing care. The Hospital-Based Clinician will be responsible for patient engagement, clinical intervention, care coordination, and follow-up outpatient support.

Shift: Part time; 1-3 12-hour shifts/week from 11am-11pm, weekends included.

 

DUTIES AND RESPONSIBILITIES:

  • Completes biopsychosocial mental health assessments
  • Develops and reviews treatment plans, goals and objectives
  • Completes discharge summaries for patients, including continuing care plan
  • Coordinates with the primary clinical team to meet patient needs
  • Administers clinical interventions, with particular focus on crisis intervention and motivational interviewing
  • Manages individual patient panel, including daily review, coordination with other staff, and care gap review
  • Engages in supervision with a licensed professional
  • Engages in interdisciplinary staffing to improve patient outcomes
  • Enters all required data into electronic health record system
  • Completes comprehensive chart reviews
  • Maintains patient confidentiality according to HIPAA and organization requirements
  • Completes all assigned staff trainings and courses
  • Maintains professional licensure and credentialing requirements
  • Performs other related duties as assigned by management

Qualifications

QUALIFICATIONS:

Education and Licensure: Master’s degree in social work, counseling, or another related field required (Licensure as LSW or LPC preferred with requirement to obtain licensure). Bachelor’s degree with significant clinical experience will be considered.

Experience: Direct clinical care experience preferred, preferably working with individuals experiencing homelessness, substance use, mental illness, and other underserved populations

  • Knowledge of utilizing trauma-informed clinical models
  • Familiarity with community support systems, health care and human resources, mental health service delivery
