GENERAL FUNCTION
The Community Health Worker (CHW) serves as an integral member of the Social Responsibility & Community Impact Family Institute Program. Under the direction and supervision of the Association Director of Family & Community Engagement, the Community Health Workers’ primary responsibility will be focused on assisting families within the Project Connect Adult Education and Workforce Development programs, connecting them to YMCA programing, resources and holistic two generational support. The Community Health Worker will work directly with the individual, families, and community partners with the intention of improving mental and physical health outcomes for families served by the YMCA of Greater Indianapolis.
KNOW HOW
A high school diploma, or its equivalent, is required. Must have existing Community Health Worker certification or agree to complete Community Health Worker training provided by the YMCA and achieve certification within first 6 months of hire. Outstanding communication skills, the ability to determine and set priorities, manage work schedules, maintain required documentation for program, and handle multiple tasks appropriately in a fast-paced work environment. The incumbent should possess a high level of initiative, professionalism, and a positive attitude and enjoy working in and creating a solid team environment. One year of experience leading/facilitating programs and working with special populations is preferred. Verbally provide clear concise directions and visually inspect classroom for safety. Ability to maintain confidentiality of program participants. Must be willing to complete YMCA provided trainings and certifications in HIPAA, CPR AED, First Aid, Safety & Risk Management, Child Abuse Prevention and New Employee Orientation the ability to obtain within 60 days of employment.
PRINCIPLE RESPONSIBILITIES
- Assist with strategizing and implementing programs to reduce barriers to access and improve overall health to the population served.
- Having relationship building skills to connect with families
- Engage parents in program design and implementation
- Assist with identifying and addressing individual and community needs around health education and access to resources.
- Assist with interpreting data from assessments to identify appropriate next steps for program sustainability in identified communities.
- Provides community resource referrals, participant advocacy, support, outreach, and follow-up.
- Provide instruction to program participants in effective and meaningful ways.
- Develop and maintain relationships that promote the building of small communities.
- Create a motivating environment that is friendly and non-competitive.
- Respond to participants and staff in a timely manner.
- Maintain Continuing Education Credits and other trainings to keep all certifications current.
- Conduct self in accordance with the YMCA Core Values; Caring, Honesty, Respect, Responsibility.
- Actively participate in the annual campaign.
- Attend all staff meetings and trainings as designated by supervisor.
- Enter data into appropriate data system and securely access/utilize/store data in accordance with privacy practices
- Available to lead Family Institute programming in the evening and at multiple sites/locations.
- Assist with other projects or activities as needed.
PHYSICAL AND MENTAL REQUIREMENTS
- Ability to stand and perform repetitive movements for extended periods of time.
- Ability to bend at the waist and knees.
- Ability to lift 40 pounds and transport equipment.
- Ability to visually inspect equipment and complete preventative maintenance checklist as required.
- Ability to communicate effectively with participants, staff and volunteers.
- Ability to demonstrate and lead activities.
- Ability to solve problems/address participant concerns and use sound judgment even when distracted by noise and activity.
- Ability to observe and promote safety and security procedures.
- Ability to physically and mentally respond to emergencies and administer CPR and First Aid effectively.

- Develop relationships that promote the building of small communities.
- Recommend participation in a variety of YMCA programs.
- Support our YMCA annual campaign by inviting members to participate.

- Provide an environment in which valuing diversity is encouraged.

- Evidence of maintenance and/or increased attendance of programs.
- Achievement of program quality metrics.
- Achievement of positive feedback as indicated by verbal comments and through written program evaluations.
