Job Summary:
The Home Health Care Nurse provides high-quality, patient-centered nursing care to individuals in their homes. This role involves assessing patient needs, developing and implementing care plans, and coordinating with other healthcare professionals to ensure optimal patient outcomes. The Home Health Care Nurse plays a crucial role in promoting the health and well-being of patients while fostering independence and comfort in their environment.
Responsibilities:
- Conducts comprehensive assessments of patient's medical, physical, and psychosocial needs
- Monitors and evaluates patients' health status, recognizing changes and adjustments in care as necessary
- Develops individualized care plans in collaboration with patients, families, and the healthcare team, detailing appropriate interventions and timelines
- Reviews and updates care plans regularly based on patient progress and changing needs
- Administers medications, treatments, and therapies as prescribed by healthcare providers
- Provides wound care, IV therapy, and other specialized nursing procedures in patients' homes
- Educates patients and their families on disease management, medication adherence, and health maintenance
- Offers emotional support and counseling to patients and families, addressing concerns and helping them navigate the healthcare system
- Collaborates with physicians, therapists, and other healthcare professionals to ensure integrated care delivery
- Communicates effectively with team members and provides updates on patient status and care needs
- Maintains accurate and timely documentation of patient assessments, interventions, and outcomes in compliance with agency policies and regulations
- Completes necessary paperwork for insurance and reimbursement processes
- Participates in quality improvement initiatives to enhance patient care and safety
- Follows infection control and safety protocols to ensure a safe environment for patients and staff
- Performs all other tasks as required by management
