Position Title:                    Nurse Practitioner      

Department:                       Primary Care

Supervisor:                        Medical Director

Status / Hours per week:  Exempt – Fulltime

Primary Function: 

Under the general direction of the Medical Director, or Physician, provide health maintenance, episodic illness, and chronic disease patient care to patients.

Duties & Responsibilities:

  • Provide primary medical management for patients through collaboration with a physician.
  • Delivers patient care and patient education in a manner that is appropriate to patient’s age, physical ability, educational level.
  • Conduct health assessments and evaluate the needs of patients in the primary care clinic.
  • Conduct physical exams, order and/or perform laboratory and diagnostic tests, and make clinical judgments regarding results.
  • Perform minor procedures independently.
  • Participate in the development of a comprehensive plan of care for each patient.
  • Initiate appropriate consultation and referrals.
  • Provide health information and teaching with emphasis on prevention and curative treatment.
  • Act as primary care resource to the team and provide in-services for staff as required.
  • Participate in teaching activities with nurses, nurse practitioner, and medical students.
  • Document History & Physical, assessment and treatment intervention in medical record.
  • Participate in committees for quality of care, standards of care, policies and procedures, and other program development activities.
  • Maintain current standards of practice and seek professional development.
  • Participate in regularly scheduled clinical care, team and department meetings.
  • Perform other duties assigned supervisor as required.  

 

 

Qualifications

Minimum Basic Knowledge:

  • Current certification and licensure as Nurse Practitioner in the Commonwealth of Massachusetts.

Experience & Qualifications:

  • Current certification and licensure as a Nurse Practitioner in the Commonwealth of Massachusetts.
  • Ability to maintain patient confidentiality at all times as required by the health center and regulations.
  • Experience working with two or more of neonatal, pediatric/adolescent, adult, and geriatric populations.
  • Ability to communicate clearly with patients and with staff.
  • Strong leadership, interpersonal, teaching, and organizational skills.
  • Willingness to work evenings and weekends as required.
  • Proficient in Spanish, Portuguese, and/or Cape Verdean desirable.

Independent Action:

  • Ability to work independently with appropriate assessment for referral.

Supervisory Responsibility:

  • Clinical supervision to nursing staff.
  • Supervision to nursing and nurse practitioner students.

Level 4 [Authorized to access full health information.  UCHC position and job responsibilities involve the provision of patient care and working as a clinical team member.  Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance].
