Position Summary

The role of the Care Coordinator – Medical Assistant reports to the RN – Clinical Manager in direct partnership of the Director of Practice Transformation. This position is responsible for the coordination of practice transformation initiatives for a group of high-risk patients within primary care locations. This position is responsible for providing direct patient care, assisting physicians with procedures, and assisting with front office duties, as needed.

 

Essential Position Responsibilities

 

  • Provide patients with timely and accurate reminders related to preventative care services
  • Provide pre-visit planning for high-risk patients to include gaps in care such as immunizations and preventive screening exams
  • Complete provider notification of gaps in care for the individual patient
  • Provide comprehensive care management to improve patient and practice outcomes
  • Partner with co-workers to provide education regarding specialty medicine practices
  • Triage patient needs, assign referrals, and assist patients as needed within the scope of a medical assistant
  • Maintain accurate documentation of referrals, specialist visits, and ED/Hospital stays and document interventions
  • Review monthly dashboards and data to conduct and participate in data analysis process
  • Complete additional duties as assigned within the scope of the medical assistant care coordinator practice

 

 Qualifications

 

  • Education: High school diploma or GED.
  • Licensure and Certifications: Certified Medical Assistant or Registered Medical Assistant, health coaching experience preferred.
  • Experience: Minimum 3 years’ in healthcare experience required.

 

Work Environment: This position will spend 20% of the time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50 lbs. This position will spend 80% of time sitting while performing work in a standard office environment. This position serves a culturally and linguistically diverse patient population. This position has exposure to communicable diseases and bodily fluids.
