- Clinical Pharmacist Duties:
- Assess the status of the patient’s health problems and determine whether the prescribed medications are optimally meeting the patient’s needs and goals of care.
- Evaluate the appropriateness and effectiveness of the patient’s medications.
- Recognize untreated health problems that could be improved or resolved with appropriate medication therapy.
- Follow the patient’s progress to determine the effects of the patient’s medications on his or her health.
- Consult with the patient’s physicians and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals.
- Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in performing patient care activities in collaboration with other members of the health care team.Â
- Advise the patient on how to organize and how best take medications.
- Support the health care team’s efforts to educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunizations.
- Refer the patient to his or her physician or other health professionals to address specific health, wellness, or social services concerns as they arise.
- Administer medications per company protocol.
- Participation in Quality Assurance projects related to the PrimaryOne Health Pharmacy Program and the 340B program.
- Maintain knowledge of latest pharmacy products and all governing laws.
- Maintain contact with other department heads, medical staff, nursing staff and others to determine needs, resolve problems, improve processes and promote effective drug therapy.
- Clinical Pharmacy Lead Duties:
- Oversee clinical leadership within clinical pharmacy programming, including collaborate with other organization department stakeholders to ensure clinical operations and services area maintained and industry best practices are supported.Â
- Gives oversight toto, improvement, and sustainability of clinical pharmacy services, internal pharmacy protocols, and collaborative practice agreements.Â
- Provide monitoring and insight to the Chief Clinical Officer to achieve timely and accurate productivity, quality clinical indicators for standard mandatory reporting, and the corporate scorecard and patient satisfaction.Â
- Provide monitoring and oversight ensuring timely completion of service information into the EHR including all administrative tasks.Â
- Manage all employed and contracted clinical pharmacists, including pharmacist residents
- Manage clinical pharmacists scheduling, and assign/manage inbox and patient panel coverage (e.g., for PTO)Â
- Serve as the pharmacist resident site coordinator to organize resident clinic schedules and clinic-based projectsÂ
- Conduct routine review discussions for clinical pharmacists at their site in accordance with the Peer Review Committee Guidelines
- Conduct the annual performance review process for clinical pharmacists (seek support from Chief Clinical Officer if necessary)Â
- Coordinate the recruitment, onboarding, and training of new clinical pharmacists and residents (including the introductory peer review of charts of new hires) as well as the ongoing training, workflow, and resource development for clinical pharmacistsÂ
- Serve on the applicable clinical lead committees/teams (e.g., lead practitioner, peer review, clinical management)Â
- Coordinate with credentialing and billing to ensure compliance and optimal reimbursement for pharmacist services
- Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors and patients.
- Other duties or special projects as assigned. Occasional travel required.
Successful candidates must possess a Doctorate degree in Pharmacy, Registered Pharmacist. Must be providing patient care in a practitioner capacity. Minimum of two years of clinical pharmacy practice or related experience. Previous involvement in developing clinical pharmacy services. Demonstrated experience in providing technical or administrative leadership among peer groups.Â
