Description

We are looking for full-time physician to join our growing team in Tifton.  This position will work in both Tifton & Albany Georgia.  Most of the work is in Tifton however.

The physician will lead a team of Nurse Practitioners who are embedded in skilled nursing facilities through the region. The physician will be a key part of our structured, collaborative care model that is transforming the delivery of healthcare in post-acute care settings and that improves clinical outcomes and patient experience.

Job Responsibilities

  • Responsible for admissions and for executing the post discharge plan of care
  • Responsible for ensuring safe discharge to home from the skilled nursing facilities
  • Provide a true continuum of care to our patients alongside our facility partners
  • Oversees nurse practitioners providing transitional care services in skilled nursing facilities
  • On-Call is remote and on a rotation with other MD/DO within the company.

Qualifications

Minimum Qualifications

  • MD or DO
  • Minimum of 3+ years of experience working with geriatric population - preferred
  • Board certified or board eligible in internal medicine, family practice or geriatric medicine
  • Ability to lead and work in a team
  • Excellent communication (written and verbal), analytical and problem-solving skills
  • Entrepreneurial mindset with ambition to transform post-acute care delivery
