•   Medical degree (MD or DO) from an accredited medical school required
•   Active, unrestricted MD/DO license or ability to obtain one required
•   Board certification or eligibility in Family Medicine, Internal Medicine, or Emergency Medicine required
•   Must show/be able to obtain active Drug Enforcement Administration (DEA) registration with prescriptive authority
•   Ability to pass credentialing and background check
