JOB PURPOSE:

Ensure delivery of services to aide in the prevention of homelessness by promoting and developing awareness of the program and conducting full eligibility screenings to potential program participants.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

 

Develop effective, trusting relationships with program participants by utilizing trauma informed care and strength base approach.  Model positive interactions, verbal guidance, and effective communication.

 

Conduct outreach and networking activities in the community; such as stand-downs, tent cities, bus stations, shelters, under bridges etc. Provide presentations at various community groups within a geographically dispersed area. Promote the success and reputation of the Supportive Services for Housing Programs.

 

Develop and maintain comprehensive outreach/working relationship with landlords, property management teams, and community partners on the participants’ behalf to develop workable plans to obtain and/or maintain housing.

 

Conduct initial eligibility screening of individuals and families seeking program assistance. Provide general information and community referrals to individuals not meeting program eligibility requirements and/or individuals not scheduled with a Case Manager due to program capacity.

 

Set up initial client file for clients being assigned to Case Manager.

 

Prepare reports as requested by Program Director.

 

Document weekly or daily case notes; submit comprehensive participant files, complete reports as needed; and enter documentation into HMIS system. Prepare necessary incident reports.

 

Maintain on-going communication with multi-disciplinary team across multiple programs to assist participants in obtaining and maintaining permanent housing, services to meet needs, and promote stability

 

Demonstrate exceptional customer service, in everything you do, by placing the child, family, Veteran or client first to support our mission to “Empower people to build better lives for themselves, their families and their communities.”

                                                                                                                                                                                   

ESSENTIAL QUALIFICATIONS: 

 

EDUCATION: High School Diploma or equivalent.

 

EXPERIENCE: 1+ years in a customer service focused environment. Experience with homeless, low income population is a plus.

 

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

 

LICENSES: Driver’s License with clear record.

 

VEHICLE: Must have daily use of a vehicle without prior notice with current auto insurance. Up to 70% travel within geographical region and surrounding areas as needed.

                                                                                                

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
