Program and Role Summary:
The primary focus of the Social Worker II is to work in the capacity as a Social Worker II will function as an Educational Liaison of the time and will facilitate educational support services for adoption and guardianship system-involved youth based on their individual needs in Contra Costa County.
This position is budgeted for 32-34 hours/week, with a varied schedule. Additionally, this role is eligible for a $2,000 sign-on bonus, paid in two installments.
Primary Responsibilities:

The Educational Liaison will work closely with the youth caregivers to assess the youth’s educational needs and provide services such as: referrals to tutoring; on-site or virtual tutoring as needed; referrals to academic enrichment activities; an onsite resource library and referrals to a local community library that offers written and audio educational materials.

The Educational Liaison will advocate for caregivers and youth and their families to help obtain 504 plans and Individual Education Plans (IEPs) as well as other educational supports.

Be a consultant to the youth, youth caregivers, and the County’s adoption and guardianship units and provide the team with solutions. Offer family specific curriculum and/or referrals to Wayfinder’s wide array of psycho-educational trainings, workshops, and support groups that can positively impact the emotional well-being of the youth.

Participate in IEP Meetings when invited by the caregiver or county staff to for our post adoption and Kinship families.

Educational Liaison to serve both pre and post adoptive homes and legal guardians. The Educational Liaison will effectively manage a caseload that is referred to by the CFS Adoption staff. Wayfinder’s Educational Liaison will aim to serve a minimum of 20- 30 unduplicated youth and their families annually. This caseload can be negotiated with County staff.

The Educational Liaison can work onsite in EHSD’s Central office a minimum of three days a week and/or as requested by county staff.
