Description

Key Responsibilities

  • Provide clinical guidance and support to case management team in the development and implementation of service plans.
  • Facilitate regular case conferencing meetings, integrating input from program staff, mental health professionals, and community partners to coordinate care for persons experiencing homelessness (PEH).
  • Conduct biopsychosocial assessments and assist in identifying barriers to stability, including mental health, substance use, housing, and employment challenges.
  • Maintain accurate clinical documentation in accordance with professional and ethical standards.
  • Collaborate with internal departments and external agencies to ensure continuity of care and access to wraparound services.
  • Support program outcomes through data collection, progress tracking, and outcome reporting.
  • Provide on-the-ground crisis assessment, de-escalation services, and safety planning for clients in acute distress.
  • Provide direct support to individuals with Serious Mental Illness (SMI) using a range of therapeutic modalities tailored to individual needs, including cognitive-behavioral techniques, trauma-informed care, person-centered approaches, and other evidence-based practice.
  • Monitor client progress and adjust interventions as needed, ensuring services remain aligned with best practices and client needs

Qualifications

  • Registered Associate Social Worker (ASW) in the State of California.
  • Experience working with PEH and understanding of trauma-informed care models.
  • Demonstrated ability to lead interdisciplinary team meetings and promote collaboration.
  • Strong organizational, communication, and documentation skills.
  • A personal relationship with Jesus Christ and a heart for ministry with vulnerable populations.

 

Experience:

  • Minimum of [X] years working in a nonprofit, ministry, or human services environment
  • Experience working with individuals facing homelessness, trauma, or addiction preferred

Skills & Abilities:

  • Ability to function effectively in a dynamic, faith-based environment
  • Strong communication and administrative skills
  • Capable of maintaining emotional resilience in challenging client situations

Work Environment and Physical Demands

  • This role may require walking, standing, lifting up to 25 lbs, and working irregular hours, including evenings or weekends.
  • Environment includes residential program sites and client-facing spaces.
