Summary: Provide consultative medication management and nursing services as defined in the regulations governing the CES Waiver and ADDT programs. Training and oversight of Direct Support Professionals (DSP) performing delegated nursing tasks.

Essential Duties and Responsibilities: include the following. Other duties may be assigned.

Duties and Responsibilities:

  1. Training and oversight of DSPs who provide medical support services as defined under Medicaid Waiver nursing consultation in community care settings.
  2. Performs and documents medication checks, as defined in an Individuals Person-Centered Service Plan.
  3. Provides annual training for staff performing delegated nursing tasks (i.e., tube feeding, medication administration and documentation, etc.) in accordance with the Arkansas State Board of Nursing regulations in all settings.
  4. Develops and implements individual nursing care plans based on documented assessment of individual. Individuals needing nursing services will be identified by each programs Interdisciplinary Team.
  5. Responsible for requesting, monitoring, and documenting use of prior authorization units related to providing nursing consultation services in Waiver program and nursing services in ADDT program.
  6. Participates in Interdisciplinary Team meetings and Individual Treatment Plan meetings for Individuals served with complex medical needs.
  7. Monitors the health status of assigned Individuals served and maintains required documentation.
  8. Confers with physicians and PASSES (Care Management team) on medical case management of Individuals served as needed.
  9. Implementation and oversight of the Medical Administrative Record system to formally record the administration of medicine within the care setting (i.e., home, center-based, community).
  10. Implementation and oversight of monthly tracking of PASSE HEDIS measures in ADDT and Waiver programs.
  11. Maintains current knowledge of all DPSQA, PASSE and Medicaid policies and regulations related to nursing services.
  12. Maintains documentation of services rendered in compliance with licensure standards and funding source requirements. Documentation is completed at the time service is rendered.
  13. Responsible for policy and procedure development and review to support clinical practices in adult programs and ensure compliance with governing regulations.
  14. Provides training on clinical policies and procedures including medication storage and administration, feeding tube management, seizure management, allergy management, and infection control.
  15. Responsible for ensuring that incident reports are completed accurately and filed timely with governing bodies. Monitors reports for patterns of incidents and works with Director(s) to evaluate and provide additional training and oversight.

While performing the duties of this job, employee will comply with all federal, state and local laws and regulations pertaining to Easterseals Arkansas. Employee has access to protected health information of the individuals directly served in Easterseals programs in which employee is engaged, except psychotherapy notes. Employee will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer

Education and/or Experience:

AD/Diploma from an accredited School of Nursing; OR BSN from an accredited program required. Two (2) years of community health (home health type) nursing experience preferred.

  10. Knowledge of Word, Excel, and Google.

Certificates, Licenses, Registrations:

Valid Arkansas Driver's License

Registered Nurse in good standing in the state of Arkansas required

Other Skills and Abilities:

Ability to lift and transfer individuals according to weight guidelines for lifting.
