Summary:
The ACL Dementia Program is a new Homage program designed to improve health outcomes of adults with/or showing symptoms of Alzheimer’s disease or other related dementias (ADRD) and strengthen the dementia-capable home and community-based supports to effectively coordinate care and improve the quality of life of adults living with ADRD.
We’re looking for a Family Caregiver Specialist who shares our values of service, integrity, respect, diversity, and innovation in helping us improve lives and community…one person at a time. The Family Caregiver Specialist will work closely with the Dementia Care Program Manager to work with caregivers in Snohomish County to promote and educate community members, caregivers, and staff about Dementia and Dementia Related Diseases. The Family Caregiver Specialist conducts assessments, develops Care Plans with clients, and provides ongoing support, while utilizing multiple database systems.
Essential Duties and Responsibilities:
- Become a certified regional trainer for the Dementia Friends program and conduct training sessions at multiple locations in Snohomish County
- Provide supportive services for unpaid family caregivers of people living with dementia or ADRD
- Conduct assessments to determine goals, strategies and services that target the stressors and burdens, and in coordination with the caregiver, develops a Care Plan.
- Assists caregivers in identifying alternative or informal resources when community resources are not available or not appropriate, and coordinates services with providers and resources to ensure service delivery
- Advocates on behalf of clients to ensure their needs are addressed in an appropriate and timely manner
- Establishes and maintains client files including accurate and timely documentation in keeping with program and agency standards
- Organizes caseload priorities in keeping with program and agency policy, including time frames, and workload expectations.
- Provide outreach to family caregivers, the community, and providers through contact, facilitation of presentations, and information events including Dementia specific outreach activities and training.
- Assist Dementia Program in facilitation of Memory Cafes and support groups within Snohomish County
- Work collaboratively with other staff, external dementia care partners, and stakeholders to promote the program and ensure its success
