Specialist, Care Coordination Outreach

Summary:

The Care Coordination Outreach Specialist is a vital connector between vulnerable adult community members and the services designed to support their health and well-being. This role is responsible for conducting direct outreach to individuals eligible for Homage Senior Services Care Coordination programs. Approximately 50% of the role will focus on enrolling Medicaid client referrals into the Health Home Program and scheduling their first care coordination visit. The remaining 50% will support outreach for the NSACH Community Hub Program, working collaboratively with partner agencies and community members to ensure seamless connections to care and supportive services.

Key Responsibilities:

Health Home Program Outreach (50%)

• Contact Medicaid member referrals to introduce the Health Home Program and confirm eligibility
• Provide clear, compassionate information to clients about the program’s benefits using trauma-informed, culturally competent communication
• Obtain verbal consent for enrollment and schedule the participant’s first visit with a Care Coordinator
• Accurately document outreach efforts, enrollment status, and contact outcomes in the electronic health record system
• Collaborate with Care Coordinators to ensure smooth transitions into care

Community Hub Outreach (50%)

• Conduct outreach to individuals referred through the NSACH Community Hub Program, following protocols established with partner agency
• Educate clients about available services and assist with navigation to social and health supports, including housing, food access, transportation, and more
• Work closely with community-based organizations and care coordinators to address barriers and build trust with clients
• Maintain accurate tracking and documentation of outreach efforts and outcomes

Minimum Qualifications:

• High school diploma or equivalent
• One year of experience in outreach, enrollment, case management, or health navigation
• Familiarity with Medicaid/Medicare programs and Dual Special Needs populations
• Ability to work independently and manage time across two distinct programs
• Strong written and verbal communication skills
• Familiarity with healthcare delivery systems and community resources
• Strong organizational and prioritization skills to thrive in a fast-paced environment
• Demonstrated ability to work independently while effectively coordinating with staff, clients, and community partners
• Proficiency with Microsoft Office and comfort learning data entry systems and EHRs

Absent any regulatory or contractual requirement for specific education/certification, other combinations of applicable education, training, and experience which provide the knowledge, skills, and abilities necessary to perform effectively in the position may be considered.

Preferred Qualifications:

• Associate or bachelor’s degree in social work, public health, or a human services field
• Knowledge and application of Trauma-Informed Care practices 
• Valid WA State driver’s license and current automobile insurance 
• Experience working with older adults and disabled adults

• Experience engaging with diverse communities
• Experience with Washington State’s Health Home or Community Hub models
• Bilingual or multilingual in a language widely spoken in the region (e.g., Spanish, Russian, Somali, Vietnamese)

Work Environment and Physical Demands:

• This work is typically performed either in an office setting or in senior centers or other community venues in Snohomish County
• Functions are usually performed sitting but may spend some amount of time standing
• When necessary, the ability to work in a semi-open office setting with frequent in-person interactions
• Ability to lift up to 15 lbs.
• Ability to use full range of senses as needed to complete the job's essential duties
• Work requires occasional travel away from the office during normal business hours

SUMMARY OF BENEFITS

Employees working 25 hours or more are eligible to enroll in medical, dental, vision, basic life, and critical illness insurance, with an increased cost share for employees scheduled for 25-29 hours/week. Employees working 20 hours or more per week receive 11 designated paid holidays plus 2 "floating holidays" per year, as well as paid vacation and sick time. Employees age 21 and older are able to enroll in our 401K plan upon hire and, after one year and at least 1000 hours worked, will be eligible for our matching program.
