Support with the development and implementation of Individual Service Plans (ISP) and
treatment plans in accordance with agency best practices, DDD requirements and
individual needs.
Complete risk assessment tools to evaluate the likelihood of re-offense and identify
specific treatment needs. Maintain accurate and detailed client records.
Work in collaboration with Nursing, Behavioral and Administrative supports to improve
quality of life, community integration and advocacy of supports and services.
Operate in compliance with (Health Insurance Portability and Accountability Act)
HIPAA regulations and DDD, or other applicable finding entities regulations.
Stay abreast by reviewing publications, articles and abstracts of current regulations and
trends; ensure appropriate actions are taken to assist with incorporating necessary revision
into workplace environment and in accordance with DDD standards.
Ensure presence and/or notes/data are made available to discuss and share during client
IDT meetings and monthly medical meetings.
Conduct individual/ group therapy sessions to provide support, encouragement and
education to the client supported.
Contribute to meetings as necessary by providing training to staff to ensure staff support
is appropriate and in alignment with the client’s needs and treatment plan.
Assist as needed with answering audit deficiencies, as well as with assisting Plans of
Corrective Action, and ensure program compliance to such plans.
Perform other related duties incidental to the work described herein.
