Main Duties & Responsibilities
- Conduct educational appointments with patients, prepare for and facilitate group sessions, providing patient follow-up and presenting educational workshops that focus on nutrition, diabetes, disease prevention and physical activity.
- Provides training and assistance for staff regarding health education issues.
- Acts as member of a patient care team with clinical staff and other health care providers
- Strong oral communication skills and can communicate effectively with patients, health care providers and community members.
