Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to the Patients Programs and Case Management Team.  The Reproductive and Child Health Promoter position requires someone who enjoys working independently and part of a team to support the needs of the site, patients, and RCHP supportive services team.  An ideal candidate would have an interest in reproductive health related issues, education, outreach, advocacy, and data entry.  This position is responsible for coordinating and monitoring the Reproductive Health Team’s assignments, deliveries, prenatal education, patient outreach, data entry, and reports. This position requires a self-motivated individual who pays close attention to detail and can manage multiple projects and deadlines.

*Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

 

Main Duties & Responsibilities

  • Schedules new OB appointments with appropriate provider (CNM or MD) and assigns and coordinates the expected deliveries among the assigned site Reproductive Health Team.

  • Provides an orientation of services to new prenatal patients (individual and/or group setting).
  • Completes the verification of pregnancy letters and refers patients for Kid Care, MPE, WIC, SDOH screenings, case management services, etc. as appropriate.
  • Creates and/or maintains accurate documentation and database for timely report production and distribution including the maintenance of the site EDC list and any OB statistics.

Qualifications

Education

  • High School Diploma or equivalent required
  •  Healthcare operations or administration experience preferred
  • Two years of experience working in a community health setting strongly preferred

 

Skills and Knowledge

Required:

  • Bilingual fluency (English/Spanish) Required.  (May be required to assist or work with non-English/Spanish only speaking patients and/or guests.)
