NTSOC serves southern Colorado families of children/young adults with disabilities, homebound seniors, and veterans by providing compassionate quality home care, physical therapy, CNA training, and outpatient therapy. NTSOC is looking for a compassionate nurse to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home.
Essential Functions:
- Provide one on one care to a single client in their home setting and while in the community.
- Implements the client’s physician ordered plan of care and nursing interventions while adhering to agency policies.
- Perform assessments to monitor the health status of the client.
- Notify the physician and Field Nurse Case Manager of any changes in the client’s condition.
- Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team.
- Administer medications and treatments as ordered.
- Document performed interventions and treatments.
- Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.
- Collaborate with the care team on implementation and updates to the client’s plan of care.
- Monitor client progress toward home health care goals.
