Seva Home Health is looking for LVN’s for our growing home health business serving Madera and Fresno Counties. Mileage is reimbursed and driving time is paid. This position is full time with on call rotation.

Requirements:
- 2 years LVN experience, home health preferred
- LVN license, required
- IV Therapy/Blood Draw certification required
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- Experience with Kinnser (WellSky) EMR preferred
- Experience with Allscripts preferred
Primary responsibilities will be:
- Serving as a point of contact throughout the admissions process
- Ensuring coordination of care with other team members and marketing team
- Communicating effectively with patients, family members, case managers, healthcare providers and co-workers
- Providing education to patients and family members regarding their home healthcare experience
- Providing an outstanding admissions experience for our patients
